To become a new customer with Data Facts, we will require the following information:
If you will NOT be pulling credit:
1. EMP agreements - no credit
2. SCS - Account setup sheet
For your convenience, you can view/print the Complete Setup Checklist - no credit to help you verify that you have completed all of the required data.
If you will be pulling credit:
1. EMP agreement - credit
2. SCS - Account setup sheet

All agreements must be signed by a principal of the company.
Please complete these forms and fax all required paperwork to us at (800) 759-8688, Attn: “New Account Setup”.
We will contact you within 24-48 hours with your account information and arrange a convenient time to meet with you to review the Shared Church Services suggested services for a complete background check.
